Hirugao explores the clandestine world of Japanese housewives engaging in illicit affairs while their husbands are busy at work, a phenomenon known as "heijitsu hirugao tsuma." The film centers on a woman whose life takes a drastic turn when she embarks on a forbidden romance with a married teacher. Once their secret is exposed, she faces severe societal backlash, becoming an outcast, shunned and publicly shamed for her actions.

The immediate consequences are profound and devastating. Forced to confront the severe judgment of her community, she is compelled to abandon her former identity, change her name, and relocate to a new residence, seeking to escape the stigma and start anew. Yet, this dramatic upheaval and forced fresh start are not the end of her complex journey. Hirugao delves deeper, revealing that the initial scandal and relocation are merely the beginning of a longer, more intricate struggle, promising there is much more to come in her pursuit of a new life amidst the shadows of her past.
